Most braces problems are not true emergencies. A loose bracket, poking wire or lost elastic can usually wait for a repair visit once your child is comfortable. Swelling, bleeding that will not stop, or a blow to the face needs a call right away.

Parents tend to find out something is wrong with braces at the worst possible time: at dinner, on a Saturday, or halfway through a school day. This guide walks through the problems we see most often at NC Tooth Docs Orthodontics & Kids Dentistry, what you can safely do at home, and how to tell a “call us today” problem from a “mention it at the next visit” one.

What Counts as a Real Orthodontic Emergency?

An orthodontic emergency is anything that causes significant pain, injures the soft tissue of the mouth, or puts a tooth at risk. Our emergency care page separates pediatric dental emergencies (broken or knocked-out teeth, toothaches, lost fillings) from orthodontic ones, and most orthodontic issues fall into a milder group: a loose appliance or bracket, a wire that has shifted, or general soreness after an adjustment.

Problems that need a same-day call

Call the office as soon as you can if your child has a hit to the mouth or face during sports or play, a tooth that feels very loose or looks out of place after an injury, swelling in the gums or face, bleeding that continues after gentle pressure, or pain that over-the-counter relief does not touch. If a mouth injury came with a head injury, go to the emergency room first; the braces can be dealt with afterward.

Problems that can wait for a repair visit

A single loose bracket, a wire end that is rubbing, a missing elastic tie or a band that has lifted slightly are repair problems, not emergencies. They are annoying and sometimes uncomfortable, but they rarely harm the teeth if you protect the cheek and call during office hours to book a time to fix them.

What Should You Do About a Loose or Broken Bracket?

A bracket usually comes loose because something hard or sticky pulled on it: ice, popcorn kernels, hard candy, a pen cap, or a fingernail habit. The bracket often stays threaded on the wire and slides along it like a bead.

At home

Leave the bracket on the wire if it is still attached. If it spins and rubs, press a small piece of orthodontic wax over it so the cheek has a smooth surface to rest on. If the bracket comes off completely, put it in a small bag and bring it to the repair visit.

When to call

Call us within a day or two so we can schedule a repair. One loose bracket is not urgent, but it means that tooth is no longer being guided, so a long delay can slow treatment down. Several loose brackets at once is worth a sooner call, because it usually points to a food or habit that needs to change.

How Do You Fix a Wire That Is Poking Your Cheek?

As teeth move, the end of the archwire can slide out past the last bracket and poke the back of the cheek. It is one of the most common calls we get, and it is usually easy to calm down at home.

Covering the end

Dry the area with a tissue, then roll a pea-sized ball of orthodontic wax and press it firmly over the wire end. The wax needs a dry surface to stick. It will wear away while eating, so keep extra on hand and replace it as needed.

Moving the wire

If a wire has shifted out of place and is irritating the mouth, our emergency guidance is to try adjusting it with clean tweezers that have been sterilized with alcohol. Use the eraser end of a pencil to gently push a poking tie wire back flat against the tooth. Never cut a wire with household tools. If the wire will not budge, cover it with wax and call us to have it trimmed properly.

What If an Appliance, Band or Expander Comes Loose?

Many children wear more than brackets. The orthodontic appliances we use include expanders, Herbst appliances, separators, elastics and retainers, and each can loosen in its own way.

Bands and appliances cemented to molars

If a band or a cemented appliance lifts off a back tooth, carefully remove the loose piece if it comes out easily, or gently guide it back into place, and call us right away to book a repair. Do not let your child keep chewing on a loose appliance, because it can bend and become harder to fix.

Separators and elastics

A separator that falls out before a banding appointment is worth a quick call so we know whether to replace it. A broken rubber band is not an emergency; replace it from your supply and keep wearing elastics as prescribed, since they only work when they are worn consistently.

Lost or broken retainers

A retainer is not an emergency for your comfort, but it is time sensitive for your result. Teeth can begin to drift within days. Call us to arrange a replacement and read more about retainer types and care. If a fixed retainer wire comes loose from one tooth, do not pull on it.

How Can You Relieve Soreness After Braces Go On or After an Adjustment?

Soreness in the first few days after braces are placed, and for a day or two after each adjustment, is normal. It means the teeth are responding. Our life with braces guide recommends a warm saltwater rinse: dissolve one teaspoon of salt in eight ounces of lukewarm water, swish for a couple of minutes, and spit it out.

Food and pain relief

Soft foods help: pasta, yogurt, soup, eggs, mashed potatoes and smoothies. An over-the-counter pain reliever, used as the label directs for your child’s age, can take the edge off. Lips, cheeks and tongue can stay irritated for one to two weeks while they toughen up, and wax over the brackets that rub helps a great deal.

Loose-feeling teeth

Teeth feel a little loose during treatment because braces have to loosen them slightly to move them. That feeling fades once the teeth are in their new positions. A tooth that suddenly feels very loose after a fall is different and needs a call.

Which Braces Problems Are Urgent and Which Can Wait?

Use this table as a quick reference. When in doubt, call; we would rather hear about a small problem early than see a large one late.

What If a Clear Aligner Cracks or Gets Lost?

Families in Invisalign treatment have their own version of the broken bracket: an aligner that cracks, warps in a hot car, or disappears from a lunch tray. There are no wires to poke, but the timing still matters, because aligners only move teeth while they are worn.

A cracked or lost aligner

Call us during office hours and tell us which aligner number your child is on. Keep the previous set of aligners rather than throwing them away, because we may ask your child to wear the previous or next set while a replacement is arranged. Do not skip ahead or stop wearing aligners altogether without checking with us first.

Attachments that come off

The small tooth-colored bumps bonded to some teeth help aligners grip. If one comes off, the aligner may feel looser on that tooth. It is not urgent, but mention it when you call so we can replace it at the next visit.

How Do You Prevent Braces Emergencies in the First Place?

Most breakages we repair trace back to a handful of habits. Fixing those habits saves a lot of repair visits.

Watch the food list

Hard, sticky and chewy foods cause most broken brackets: ice, nuts, hard candy, caramel, gum, bagels and whole apples or carrots. Cut crunchy fruits and vegetables into small pieces instead of biting into them. Our post on foods to avoid with braces or aligners has a full list.

Protect braces during sports

Children can keep playing sports in braces. A braces-safe mouthguard protects both the teeth and the lips from brackets during contact. If an injury does happen on the field, our guide to knocked-out and chipped teeth covers what to do in the first few minutes.

Break chewing habits

Pens, pencils, fingernails and water bottle caps are brackets’ quiet enemies. A reminder at homework time helps more than parents expect.

Frequently Asked Questions

Is a broken bracket an emergency?

Usually not. If the bracket is still on the wire and your child is comfortable, cover it with wax and call during office hours to book a repair. It becomes more urgent when several brackets are loose or the wire is cutting the cheek.

Can I cut a poking braces wire myself?

We do not recommend it. Household clippers can leave a sharp end or let a piece fall into the mouth. Cover it with wax or gently reposition it with sterilized tweezers, then let us trim it safely.

What should I bring to a repair visit?

Bring any piece that came off, such as a bracket, band or broken retainer, in a small bag. Knowing what happened before it broke also helps us prevent a repeat.
