Silver Diamine Fluoride for Kids: What Parents Should Know

Silver diamine fluoride (SDF) is a liquid painted onto a cavity to stop it from getting worse. It contains water, silver, fluoride and ammonia, needs no drilling or numbing, and can serve as an alternative to a filling for some children. The main trade-off is that treated decay turns dark.

If your child’s pediatric dentist has mentioned SDF, you probably have questions about how it works, whether it is safe and what it will look like. It is a newer option for many parents, even though it has become a common tool in children’s dentistry. Below we explain what SDF does, which children it helps most, what to expect at the appointment and how it compares with a traditional filling.

What Is Silver Diamine Fluoride?

Silver diamine fluoride is a clear liquid that a dentist brushes directly onto a decayed area of a tooth. As our silver diamine fluoride page explains, it contains water, silver, fluoride and ammonia, and it can prevent cavities from worsening and may serve as an alternative to a dental filling.

Each ingredient has a job. The silver works against the bacteria that drive tooth decay, and the fluoride helps strengthen and remineralize the tooth surface. The ammonia keeps the solution stable. Together, they can arrest a cavity, which means the decay stops progressing even though the damaged area is not removed.

SDF is different from the fluoride varnish many children receive at checkups. Varnish is a preventive coating painted on healthy teeth to strengthen enamel. SDF is a treatment applied to an existing cavity. Our fluoride treatments overview explains how varnish fits into routine care.

Which Children Benefit Most From SDF?

SDF is especially helpful in pediatric dentistry for kids with severe dental anxiety and for children with special needs who may not be able to engage or cooperate during a traditional appointment. Because there is no drilling, no shots and no need to sit still for long, it can treat decay that would otherwise require a much more involved visit.

It is also a useful option for very young children whose cavities are found early, when a full filling appointment would be difficult. In some cases, SDF can hold decay in check until a child is older and more comfortable with treatment, or until a baby tooth is ready to come out naturally.

SDF is not the right answer for every cavity. Deep decay that has reached the nerve, a tooth that is already causing pain or infection, or a cavity in a highly visible spot where the dark stain would bother your family may call for a different approach, such as a white filling, a crown or a baby root canal. Your child’s dentist will explain which option fits each tooth.

What Happens During an SDF Treatment?

The appointment is short. The tooth is cleaned and kept dry, often with cotton rolls, and a small amount of the liquid is brushed onto the decayed area with a tiny applicator. It is left to soak in briefly, and the area is kept dry for a short time so the solution can work. There is no drilling and no need for numbing, so most children tolerate it easily.

Because the treatment is so quick, it pairs well with other comfort measures. At NC Tooth Docs Orthodontics & Kids Dentistry, our pediatric team aims to make every visit as stress-free as possible and offers nitrous oxide, or laughing gas, when necessary to help reduce anxiety in our youngest patients. Learn more on our nitrous oxide page.

Does SDF Need to Be Repeated?

Sometimes. Depending on the cavity, your child’s dentist may recommend reapplying SDF at a later visit and will check the treated teeth at regular exams to make sure the decay stays arrested.

How Does SDF Compare With a Filling?

A filling removes the decayed part of the tooth and replaces it with a tooth-colored material, restoring the shape of the tooth. SDF leaves the tooth structure in place and stops the decay from spreading. Each has a role, and the table below summarizes the main differences.

Question Silver diamine fluoride White filling
How it works Arrests decay with silver and fluoride Removes decay and restores the tooth
Drilling or numbing Not needed Usually needed
Appearance Treated decay turns dark or black Tooth-colored
Visit length Very short Longer appointment
Often chosen for Anxious or very young children, special needs Children who can sit for treatment, visible teeth

The staining is the biggest consideration for most families. The decayed area turns dark brown or black once SDF is applied, while healthy enamel around it is not affected. On back baby teeth, many parents find this an acceptable trade for avoiding a difficult appointment. On front teeth, families often weigh it more carefully. The stain can also mark skin or clothing on contact, so the team protects the area during treatment. If a filling is placed later, a tooth-colored restoration can cover the treated spot. Our white fillings page explains that option.

Is Silver Diamine Fluoride Safe for Kids?

SDF is used in a very small amount, applied only to the affected tooth. Like any dental treatment, it is not for everyone. Children with a silver allergy should not receive it, and your dentist will review your child’s health history first. Some children notice a metallic taste for a short time afterward.

Dr. Amanda is a board-certified pediatric dentist and a Diplomate of the American Board of Pediatric Dentistry. She understands that all pediatric patients have different needs and talks with parents about every option, including when SDF makes sense and when another treatment is better. Our post on whether cavities in baby teeth need fillings covers why treating baby-tooth decay matters.

How Can You Help Prevent New Cavities After SDF?

SDF stops existing decay, but it does not protect the rest of the mouth from new cavities. Prevention is a major part of pediatric dentistry at NC Tooth Docs, and our prevention services include routine exams, dental cleanings and hygiene, sealants and fluoride treatments. A child who needed SDF is often a child who benefits from a little extra attention to these basics.

At home, brushing twice a day with a fluoride-containing toothpaste and flossing regularly make the biggest difference. A healthy diet helps too, especially limiting frequent sugary snacks and drinks between meals. Fluoride varnish can be applied twice a year as part of your child’s regular dental care, and fluoride treatments are most effective when combined with good home care, a healthy diet and a dental appointment every six months.

Sealants are another option for the chewing surfaces of back teeth, where grooves trap food and plaque. Our post on whether dental sealants are worth it for kids explains how they work. Regular checkups also let the dentist confirm that any SDF-treated teeth are staying stable.

Frequently Asked Questions

Does SDF hurt?

No. It is brushed onto the tooth without drilling or shots. Some children notice a brief metallic taste.

Will the black stain go away?

The stain on treated decay is permanent on that tooth surface. On baby teeth, it leaves when the tooth falls out. A filling or crown can cover it if needed.

Can SDF be used on permanent teeth?

It can be used on some permanent teeth. Your child’s dentist will explain whether it fits a specific cavity.

Does insurance cover SDF?

Coverage depends on your plan. NC Tooth Docs accepts all PPO insurance plans as well as Medicaid, and our team can help you check your benefits.

NC Tooth Docs are absolutely amazing. I've been taking my daughter here for years and we always need to end up needing braces. They are so respectful kind and patient when it comes to working with children. My daughter truly has absolutely no fear of going to the dentist because of how amazing they are. Such a sweet, thorough and friendly staff!

Dr. Amanda treated 9 of my 9-year-old daughter’s teeth over 3 visits. It was a bit stressful since there were crowns and even an extraction, but my daughter handled everything very calmly. I’d also like to give special thanks for numbing before the anesthesia. I was most worried about that part, but everything went smoothly. Despite such extensive treatment, my daughter did not develop any fear of dentistry. I highly recommend both the clinic and the doctor — fast, high-quality, and very attentive.

Going to Nc Tooth Docs has been nothing short of AMAZING. My daughter got her braces there and recently removed. Dr. J is wonderful. I literally watched him treat each patient with genuine care and respect. He was very informative and transparent. We need more Dr. J's in the world. And Da'Lisa has been so outstanding. The way she bonded with my daughter is what topped it all off for me. With every visit she made us feel very lucky to have her. Even when my indecisive daughter took forever choosing a color for her braces, Da'Lisa was very patient and kind. Never made my daughter feel bothersome or rushed. She is a super star in my book and will be in yours if you ever have the pleasure of meeting. She even went out of her way to get my daughter a birthday gift. From the time you enter and are greeted by the front desk ladies, to the time you leave, NC Docs of Huntersville staff is top notch. I'd even go as far to say they are the Chick-fil-A of dentistry!!!!! Trust me, they are the ones you need!! Thank you again Dr.J and Da'Lisa!!!

Can’t recommend them enough!
